Custom Form Fields

The custom form fields can be used to create any additional questions/statements you want to ask or have read during the registration. 

 “Select” - Select which type of question/form field you would like to create.

“Label/Text/Content” - Input the questions or statement

“Available Values” - Input all the acceptable answers you would like the registrant to choose from

“Form Page” - Select which page of the registration form you would like this question to appear on             (*Default will be the first page of the registration)

“Row #” -What order the question will appear on the page

“Required” – Checking will make the specific question required to answer or checked
